The prostate is a small glandular organ that wraps around the urethra. Many men find that anal penetration pleasurably stimulates the prostate and enhances sexual pleasure. In addition, prostate secretions are responsible for the largest fraction of the liquid component of semen, and prostate fluids are important for sperm health and function.
When the prostate is removed via prostatectomy, men often end up with reduced or absent ejaculation during orgasm. This is both because there is less semen produced in the absence of the prostate and because prostatectomy can lead to retrograde ejaculation -- ejaculation backwards into the bladder instead of out of the penis. However, orgasm can still take place, even without ejaculation.
You can find out more about specific methods of prostatectomy in the A.D.A.M online medical encyclopedia, including trans-urethral resection, simple prostatectomy, and radical prostatectomy.
